*Parental Consent

The parent/guardian of the student hereby agrees to allow their son/daughter to take part in one-on-one driving lessons with a certified GMDS Driving Instructor and to partake in all the mandatory curriculum and courses that correspond with the program in which the student is enrolled.

*Paperwork

All paperwork must be filled out prior to a student beginning their program, using the true legal name of the student that matches the student’s Learner’s Permit. Please always have the student’s Learner’s Permit available; during the first lesson, the Driving Instructor is required to make a copy (photograph) for GMDS records. Instructors carry the paperwork, or it may be downloaded from our website.

*Learner’s Permit

A student who holds a learner’s permit must have the original learner’s permit (photocopy is not permitted) with them for their lessons or else the lesson will be forfeited, and the no-show fee will be charged to the student.

*Student Cancellations, Rescheduling and Rebooking Fees

If the student needs to cancel or reschedule a lesson, he/she must notify GMDS 48 hours (2 days) prior to their appointment time, there will be no fees to do so. If a student does not show up to his/her session, he/she will lose that lesson and will not be refunded. To rebook the lesson that was missed and not cancelled / rescheduled within 48 hours, there is a Rebooking Fee of $100 to schedule a replacement appointment.

*No Shows

Driving Instructors will wait 15 minutes before considering the student a no-show. There will be no exceptions. If the Driving Instructor has waited 15 minutes, and has tried unsuccessfully to contact the student, they will leave the pick-up site, and the student will lose that lesson, and will have to reschedule their lesson and will incur the $100 Rebooking Fee.

*Rescheduling Appointments

GMDS, in its sole discretion, reserves the right to reschedule a student’s appointment based on Instructor availability. Due to unforeseen circumstances, the Instructor may show up late. The Instructor might also have an emergency and, therefore, GMDS might have to reschedule an appointment at the last moment. Remember the Instructor will conduct a full lesson if they are late. GMDS will notify a student immediately if there needs to be a change. There will be no charge.

*Extra Lessons

If the student does not complete his/her lessons as scheduled, an Extra Lesson Fee of $50 per extra lesson will be charged. For instance, if you opt for 10 lessons of 60 minutes each instead of the provided 5 lessons of 120 minutes, the student will incur a total additional fee of $200 for the extra 5 lessons.

*Guarantee or Warranty

GMDS DOES NOT PROMISE, WARRANT, OR GUARANTEE THAT ANY OF ITS STUDENTS WILL PASS HIS/HER DMV ROAD TEST OR THAT THEY WILL NOT GET INTO ANY AUTOMOBILE CRASHES WHILE TAKING PART IN ANY BEHIND-THE-WHEEL TRAINING LESSONS. To the fullest extent allowed by law, GMDS disclaims any and all express and/or implied warranties related to its program, methods, processes and/or curriculum.

*Refunds

GMDS will not issue refunds once the student begins his/her behind-the-wheel lessons. GMDS, however, reserves the right to issue refunds in extraordinary circumstances on a case-by-case basis. All refund requests must be made in writing and include the specific reason(s) for the requested refund and, only if accepted and approved by GMDS management, a refund will be issued to the student on a pro-rated basis.

*Release and Waiver

The student and his/her parent/guardian hereby release, waive, & discharge all claims & potential claims against, & covenants not to sue GMDS, its parent, subsidiaries, affiliated entities, officers, employees, agents & facilities with respect to any damages, injuries or losses of any kind to the student or any vehicle or any property damage or loss to any other party arising in connection with the student’s participation in the GMDS curriculum & driving GMDS vehicle.

*Driving Instructor Development/Shadowing

As part of our ongoing Driving Instructor skill development and commitment to our level of quality and service through our lesson delivery, GMDS Instructors will periodically shadow one another's lessons. During a student’s behind-the-wheel training with GMDS, there may be a second Driving Instructor present in the vehicle as a silent partner observing the lesson.

*Additional Passengers

The parent/guardian agrees that multiple students may not be in a vehicle under the supervision of a GMDS Driving Instructor without approval from the parent who purchased the lesson. If a student or passenger is in the vehicle, they must be enrolled in our program to be allowed to be a passenger.

*Filming

Both the parent/guardian and student consent to the student being filmed during lessons. Each GMDS vehicle may be equipped with an external and/or internal video camera(s) to record the student’s driving performance and events on the road. Videos that show students' faces will not be distributed externally without prior consent.

*DANGER

I UNDERSTAND THAT TEACHING STUDENTS TO DRIVE IS DANGEROUS AND FLUID. THE INSTRUCTOR WILL TAKE EVERY PRECAUTION AVAILABLE TO KEEP THE STUDENT AND VEHICLE SAFE. HOWEVER, THE STUDENT IS OPERATING THE VEHICLE IN CONTROL, AND ULTIMATELY RESPONSIBLE FOR ANYTHING THAT HAPPENS. ACCIDENTS ARE ALWAYS A DISTINCT POSSIBILITY.

*The undersigned agrees and understands all of the above terms laid out in this agreement.

*A. Possess a valid driver permit/license and shall comply with all indicated restrictions; and

*B. Always operate the vehicle in a safe manner, observing all traffic laws; and

*C. Always obey the directions and commands of the GMDS Driving Instructor and authorized representatives; and

*D. Be responsible for any fines or penalties involving the vehicle for parking, traffic or other violation of any traffic laws incurred as a result of Driver’s operation or use of a vehicle.

*E. Be listed as the driver of record on any police and/or incident reports that are prepared and/or filed because of an accident/incident that occurs while the student is behind the wheel of the vehicle during a behind-the-wheel training session or lesson and accept any consequential impact on their driving record that occurs as a result of being involved in any such accident/incident.
